How do I register my child for Kindergarten?

Ballard Community Schools require the use of our online registration system for all new and returning students.  Please choose the registration process below that applies to your family.

New Family to the District

You do not have any children currently enrolled at Ballard or any of our local preschool programs

Current Ballard Family

You have a child enrolled at Ballard Schools (EK-12) or one of the local preschool programs

For all families who have had a child enrolled in Ballard Community Schools or one of our 4 year old preschool programs, please log into your Infinite Campus Parent Portal account and select Online Registration from the menu.  You will be able to register any existing students and add your new Kindergarten student to your existing household.

What else is required to complete my child’s Kindergarten registration?

The following documents are required to be submitted before a student’s registration is complete.  Please use the Kindergarten Registration Checklist to organize your child’s paperwork.

District Proof of Residency

All new students to Ballard Community Schools must provide proper documentation establishing residency in the district. The name and address on these documents must match the name and address of the parent or legal guardian of the student(s) being registered. Residency at the physical address is required.

  1. A copy of your most recent property tax bill
  2. A copy of a recent mortgage statement
  3. A utility bill for the last or current month, mailed to your Ballard residence

A copy of the settlement statement from closing or the warranty deed

A copy of the purchase contract that states the Ballard address, possession/closing date and signature of parent/guardian

A copy of your signed lease agreement with terms listed.  The agreement MUST contain property owner’s name, address and signature; and name and signature of parent/guardian

Utility bill for the last or current month, mailed to your Ballard residence

Completed and Notarized Co-residency Form

Proof of your residence at that address. (ie: bank statement, insurance statement, utility bill or any other bill, pay stub from your current employer, etc.) with your name and Ballard address clearly listed and from the last or current month

Verification of residency for the family with whom you are residing with:

(proof of residency can be in the form of: mortgage statement, lease agreement or utility bill.  NO PO Box accepted)

Required Health Documents

The Iowa Department of Public Health requires every school to have an up to date and completed certificate of immunization on file for each student enrolled.  These immunization requirements must be on file with the school nurse by the time the student starts school as this is required by state law.  For more details on specific information regarding each screening or vaccination, please talk to your medical care provider or school nurse.

Please have the following forms filled out by a healthcare provider to be submitted for enrollment:

Iowa law requires a parent to verify proper immunization against diphtheria, tetanus, pertussis, polio, measles, mumps, rubella, hepatitis B and varicella to enroll in school.  At least one dose of each immunization must be given before starting school.  An immunization or exemption certificate must be on file with the school nurse by the time the student starts school as this is required by state law and Board Policy 507.01.

Please review the immunization requirements per the Iowa Department of Public Health as the State of Iowa has passed some new requirements in recent years.  The state of Iowa requires students entering 7th grade to have both a Tdap vaccine and a meningitis vaccine.  For those students entering 12th grade, you will need to show proof of two doses of meningitis vaccine.  The meningitis vaccine must be received before the first day of 7th or 12th grade and an updated immunization record must be to the school nurse before the first day of school.

If you have specific questions or need more information on immunizations or any of the health screening, please talk to your medical care provider or school nurse.  Your health care provider should be able to provide you with the appropriate immunization certificate however if you need a blank certificate of immunization or exemption form, please click here.

Required by Iowa law, dental screenings are to be completed for all students entering kindergarten and ninth grade.  Students entering kindergarten must have had a screening completed no sooner than 3 years of age and no later than four months after school enrollment.  It can be completed by a licensed doctor, advanced registered nurse practitioner, physician assistant, nurse, dental hygienist, or a dentist.  Students entering ninth grade must have proof of a dental screening by a dentist or dental hygienist completed no earlier than one year prior to enrollment and no later than four months after enrollment.  The results of this screening need to be submitted to your school nurse on the designated dental screening form which can be located on the Iowa Department of Public Health‘s website.  One may also locate a dental screen exemption on the same site.

The State of Iowa requires all incoming kindergarten and third grade students to have a vision screening prior to the first day of school.  This exam can be completed by a licensed eye doctor or optometrist, your child’s health care provider at the time of his or her school physical, a free clinic, KidSight, or a county health department. A screening will be valid if it is done up to 1 year prior to enrollment and no later than 6 months after enrollment.  The results of this screening need to be submitted to your school nurse on the designated vision screening form which can be located on the Iowa Department of Public Health‘s website.

All incoming Early Kindergarten and Kindergarten students are requried to have an up to date physical on file. Please use the attacehed form or submit a printout from your family’s physician.
